Transitional Care

Trusted Transitional Care program for a seamless path to recovery.

Transitional care patients are cared for in a separate unit on the third floor. Our transitional care interdisciplinary team provides high quality nursing care and rehabilitation while focusing on helping each patient achieve their highest level of function and independence as possible. Transitional care patients require daily rehabilitation and skilled nursing services, and are admitted to the unit with a reasonable expectation for discharge.

Our large rehabilitation gym is located on the Transitional Care Unit providing Physical, Occupational, and Speech Therapy, and allowing the rehabilitation staff to work more closely with the nursing staff in meeting the therapy goals of each patient.

Meals and refreshments are served in the Transitional Care Dining Room and patients are encouraged to enjoy their meals in the dining room. A spacious lounge on the TCU allows family members and friends to visit with patients in a comfortable setting on the Transitional Care Unit.
