Transitional Care Unit (TCU)

Transitional care patients are cared for in a separate unit on the third floor.  Our transitional care interdisciplinary team provides high quality nursing care and rehabilitation while focusing on helping each patient achieve their highest level of function and independence as possible.  Transitional care patients require daily rehabilitation and skilled nursing services, and are admitted to the unit with a reasonable expectation for discharge.

Three nutritious meals and refreshments are served daily in the dining room located on the unit. Family members can enjoy meals with their loved ones.  Spacious lounges and outdoor patios are available for the enjoyment of our patients and their families.
